Cityblock Health and Mercy Health Partner to Transform Medicaid Care in Ohio from HIT Jasmine Pennic

Cityblock Health, a value-based provider specializing in the complex needs of the Medicaid population, and Mercy Health, part of the Bon Secours Mercy Health system, announced a strategic collaboration in the greater Cincinnati and Toledo areas of Ohio.

– Cityblock will provide care extension services to complement the primary care services already delivered by Mercy Health’s existing provider network. 

Wrap-Around Care Coordination

This wrap-around care coordination model will be offered 24/7 and spans several critical modalities:

  • Primary and urgent care extension
  • Behavioral health and social care
  • Care delivered via in-person and virtual modalities

Dr. John Luellen, State President, Ohio, Mercy Health, noted that this collaboration “allows us to better meet the evolving needs of the communities we serve” by “expanding access to compassionate, high-quality care for Medicaid patients, meeting them where they are and ensuring their full spectrum of needs is addressed”.

The partnership unites two mission-aligned organizations with a shared commitment to improving outcomes for underserved communities in Ohio. Mercy Health is one of the largest non-profit Catholic health systems in the United States.

The integrated approach aims to improve outcomes and lower costs by focusing on systemic challenges:

  • Effectively engaging hard-to-reach patients.
  • Unlocking access to care.
  • Addressing crucial social needs, such as food and housing.
  • Reducing avoidable emergency room visits and hospitalizations.
